Condominium Act Forms for Owners
The Ontario Ministry of Government and Consumer Services has delegated the hosting and maintenance of various Condominium Act forms to the Condominium Authority of Ontario (CAO) as of January 2020. These forms include:
Forms relating to Records and Request for Records
- Submissions to include material in the notice of meeting, for owner meetings.
- Forms relating to Meetings and Voting
- Other Condo Act forms

Please note that the “Request for Records” form prescribed by the Condominium Act is a mandatory form and must be used as per Regulation 48/01 s 13.3 (3). Once downloaded from the CAO and completed in full, the request should be sent to:
